MAWD Application Process

Once you have decided what your career goal is and what steps need to be taken to meet your goal, you may want MAWD (Medical Assistance for Workers with Disabilities) to help with medical insurance.  If you meet the following guidelines, you can apply for services. 

 

            ** Need to be employed and receive compensation

            ** Must meet the definition of disability according to the Social

                        Security Administration

            ** Be between the ages of 16 and 65

            ** Have a gross income of less than $46,000

            ** Have assets less than $10,000 (not including your house or car)

 

You can apply for MAWD services without leaving your house.  You can call your local County Assistance Office; (DPW) Pennsylvania Department of Public Welfare by phone at 1.800.842.2020, http://www.state.pa.us, or

COMPASS (Commonwealth of Pennsylvania Access to Social Services)http://www/compass.state.pa.org.

 

Prior to completing your application you should collect some important documents that you will need to support and verify answers to questions.  Some examples are pay stubs, bank statements, cash on hand, medical information, social security numbers, and rent receipts.   The application is small, with only a few pages, and takes a short time to complete, approximately 15 to 30 minutes.

 

The application has four steps.  The following information should help you to complete your application.

 

Step One has four Sections.  The First Section is Household Members.  In this section you will need to provide information on each household member you listed on your application.  Some information you will need to supply is birthdates, social security numbers and Medicare Claim numbers.  If anyone on your list is disabled, you will need to identify and explain their disability.  You can use such items as a driver’s license, photo ID, Social Security card and medical documentation.

 

In the Second Section, Income, you will need to provide information on yourself and each household member you listed on your application.  Before you begin filling out the application, you may find the form easier to complete by gathering basic information about employers. You will need names, addresses, phone numbers, hours worked and wages (receipts or check stubs). 

 

In the Third Section, Expenses, you will need to list expenses.  Some examples of expenses are court costs, attorney fees, transportation, disability related work expenses, or any other disability related expense.  You need to provide documentation to verify each expense, such as a receipt.  If you lost a receipt, you may be able to identify the item and write a small paragraph explaining your need for that item.

 

In the Forth Section, Resources, you will need to provide information on your resources.  A few examples of resources are bank statements, savings accounts, stocks & bonds and insurance policies.  You will need to look on your application and identify those resources that apply to you and each family member listed on your application.  Some information you will need to provide includes account or insurance policy numbers and values.  You can use bank statements and insurance policies to help you.

 

In Step Two, you need to attach all documents (i.e., receipts, paragraphs of explanation) you used for verification when you were completing your application.  You may prefer to make copies to send with your application and keep the originals for your records.

 

In Step Three, you need to read the Rights & Responsibilities. Sign and date your application. 

 

In Step Four, you are almost finished.  All you have left to do is mail, fax or take your completed application and documents to your local county assistance office.  If you applied over the internet, your local county assistance office also needs copies of your documents.

 

After your county assistance office receives your application, you will be assigned a caseworker.  The caseworker will review your application and contact you if clarification or additional information is needed to determine your eligibility.
